These agencies include the Department of Veterans Administration (“VA), the Department of Housing and Urban Development (“HUD”), Federal National Mortgage Association (“FNMA”), or any other institution in connection wit the consideration or administration of assistance to you. Financial records involving your transaction will not be disclosed outside the agency or its assignees without your consent, except as required and permitted by law. You are not required to provide this information, but if you do not, your application for approval as a prospective mortgagor or borrower may be delayed or rejected. The information requested in this form is authorized by Title 38, U.S.C., Chapter 37 (if VA); by 12 U.S.C., Section 1701 et seq. (if HUD/FHA); by Title 42 U.S.C., Section 1425b (if HUD/CPD); and Title 42 U.S.C., 1471 et seq., or 7 U.S.C., 1921 et seq. (if USDA FmHA).
